Cough and body aches without fever

A fever may be absent early in an illness, may have settled, or may not develop in every person. COVID-19 and Influenza can include cough, tiredness and muscle aches, while other respiratory infections cause similar symptoms. Body aches can also follow poor sleep, strenuous activity or dehydration. Persistent widespread pain with fatigue may need a different assessment, including consideration of Fibromyalgia or Hypothyroidism. Travel, animal contact, insect exposure and unwell household contacts can change which infections a clinician considers.

Rest, drink regularly and use gentle movement if it does not increase pain. Check your temperature if you feel hot, chilled or suddenly worse, and consider staying home from close contact while respiratory symptoms are active. Follow the label for any pain medicine and check with a pharmacist if you are pregnant, have liver or kidney disease, or take other medicines. Note symptom onset, exposures, travel and any new rash, vomiting or diarrhoea.

Get urgent help for difficulty breathing, chest pain, confusion, faintness, severe weakness, inability to keep fluids down, or a new widespread rash with serious illness. Arrange medical review when aches are severe, symptoms keep worsening, cough persists, or a fever appears later. Seek earlier advice after travel to a malaria area, animal exposure, or contact with someone known to have a significant infection.
