COPD nursing diagnosis: how assessment works

A medical diagnosis identifies COPD as a disease. A nursing diagnosis describes how the disease is affecting a person and what support may help. The nurse considers breathing pattern, breath sounds, cough, phlegm, fatigue, activity tolerance, sleep, nutrition, and ability to manage medicines.

The assessment may lead to goals such as moving more comfortably, clearing secretions, using an inhaler correctly, or recognising a flare-up. The plan is individual and can change when symptoms, examination findings, or daily abilities change.

Describe your usual abilities as well as what has changed. Explain whether breathlessness limits washing, dressing, stairs, work, or sleep. Tell the nurse about cough, wheeze, chest tightness, phlegm colour or amount, smoking, and any difficulty taking medicines as prescribed.

Work through the agreed goals and practise techniques during the visit. A chronic disease review can bring nursing observations, medical treatment, and spirometry findings together. Ask for written instructions if you are uncertain how to respond to worsening symptoms.

Contact your care team when your breathing, cough, wheeze, phlegm, energy, or ability to manage daily tasks changes from your usual pattern. Emergency help is needed for severe breathlessness, confusion, fainting, blue or grey lips, or difficulty speaking.

Ask which nursing problem has been identified, what goal you are working toward, how progress will be checked, and what you should do if symptoms worsen. Ask whether spirometry or a medication review is needed.
