If the discomfort clearly followed a minor exercise overload and there are no warning signs, relative rest and comfortable ankle movement may be reasonable while you arrange review if it does not settle.
A clinician may inspect both legs, assess pulses, sensation, strength, the Achilles tendon, and pain with movement. Further testing depends on the findings. Bring a list of medicines and explain exactly when the symptoms started rather than describing them only as cramp or tightness.
When calf pain is urgent
Seek urgent medical assessment for new unexplained one-sided calf pain with swelling, warmth, redness, or marked tenderness. Call local emergency services for calf symptoms accompanied by sudden shortness of breath, chest pain, coughing blood, fainting, or severe weakness.
Immediate evaluation is also appropriate if the foot becomes cold, pale or blue, numb, or difficult to move, or if severe pain follows an injury with obvious deformity. Do not massage the calf while waiting for assessment when a clot is a concern.
