What should you know about constipated diarrhoea?

Short answer
Constipated diarrhoea can happen when loose stool passes around retained hard stool, so repeated episodes need assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Possible mechanism
- Liquid stool passing around retained hard stool
- Avoid
- Repeated anti-diarrhoeal medicine without advice when you feel blocked
What the pattern may mean
Constipation does not always produce dry, infrequent bowel movements alone. When stool builds up, liquid stool may seep around it, causing urgency, leakage or several loose stools while you still feel blocked or incompletely emptied. Alternating constipation and diarrhoea can also occur with irritable bowel syndrome.
Other possibilities include a recent stomach infection, food intolerance, coeliac disease, inflammatory bowel disease, or a medicine effect. A clinician will ask about the timing, stool appearance, pain, bloating, fever, recent travel, diet and medicines rather than assuming one cause from the stool alone.
What to do next
Drink fluids regularly and avoid taking repeated anti-diarrhoeal medicine if you feel impacted, unless a clinician advises it. Do not try to remove stool yourself. Gentle movement, regular toilet time and a gradual return to fibre may help, but increasing fibre quickly can worsen bloating when stool is retained.
Arrange a medical review if the pattern continues, keeps returning, or does not improve with simple measures. Take notes about bowel movements, leakage, abdominal symptoms and any recent antibiotics, travel or dietary changes.
When to seek medical help
Seek urgent care for severe or increasing abdominal pain, a swollen abdomen, repeated vomiting, inability to pass gas or stool, blood in the stool, black stool, faintness, or signs of dehydration such as very little urine and marked dizziness. Fever with significant abdominal pain also needs prompt assessment.
Questions for your doctor
Could retained stool explain the leakage? Do I need an examination or tests? Which fluids, fibre or bowel medicine are suitable, and which warning signs should make me stop self-care?
