What should you know about coning diastasis recti?

Short answer

Coning diastasis recti is a midline ridge or dome that can appear when the abdominal wall bears pressure; it may reflect reduced tension or coordination through the linea alba and is worth assessing if it persists, causes symptoms, or limits movement.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What coning means

Diastasis recti, also called abdominal separation, involves stretching and thinning of the connective tissue between the two sides of the rectus abdominis muscles. Coning may show up when you sit up, lift, cough, roll in bed, or hold your breath while exerting yourself. The shape alone does not show how wide the separation is or whether the abdominal wall is functioning well.

Pregnancy can place sustained load on the abdominal wall, but coning can also occur with other activities that increase pressure inside the abdomen. Some people notice an appearance change without pain; others report back discomfort, a sense of weakness, pelvic-floor symptoms, or difficulty controlling a movement. A clinician or physiotherapist can examine your breathing, trunk control, and movement rather than judging the condition by appearance alone.

What you can do

For now, reduce the effort or change the position of an exercise that repeatedly produces pronounced coning. Breathe out during the harder part of a lift, avoid breath-holding, and use a smaller range of motion when that helps you keep control. Gentle walking and comfortable daily activity are generally reasonable unless they worsen pain or other symptoms.

An individual assessment can guide a gradual programme for deep abdominal coordination, posture, breathing, and pelvic-floor function. Postnatal Physiotherapy may be useful after pregnancy or birth, especially when coning accompanies heaviness, leaking, back discomfort, or uncertainty about returning to exercise. Do not force the abdomen flat with intense bracing or continue an exercise through pain.

When to seek care

Arrange a non-urgent assessment if the ridge remains noticeable, you cannot load your trunk confidently, or symptoms such as abdominal or back pain, pelvic pressure, urine leakage, or bowel-control changes interfere with daily life. Assessment is also sensible when you are unsure whether a new lump is coning or a hernia.

Seek urgent medical help for sudden severe abdominal pain, a firm or very tender bulge that will not settle, repeated vomiting, a swollen abdomen with inability to pass stool or gas, fainting, or heavy bleeding after birth. These features need evaluation because they are not explained by coning alone.

Questions for your doctor or physiotherapist

Ask: What movement is producing the coning, and can you check my breathing and abdominal control? Is a hernia or another cause of the bulge possible? Which exercises should I modify now? How should I progress lifting, running, or sit-up movements? What symptoms should prompt an earlier review?

Questions people ask

Is coning the same as a hernia?

No. Coning is a midline change in the abdominal wall during pressure or movement, while a hernia is tissue pushing through a weak area. A persistent or painful lump needs clinical assessment.

Should you stop all exercise if coning appears?

Usually, you can modify rather than abandon activity: reduce the load or range, keep breathing, and choose movements you can control without pain. A physiotherapist can help plan progression.

Can diastasis recti be assessed after pregnancy?

Yes. A clinician can assess the abdominal wall and related symptoms at any stage after pregnancy, including how the muscles coordinate during everyday tasks and exercise.
