Congestive heart failure coughing at night

When you lie flat, fluid can shift toward the chest. If the heart is struggling to manage the returning blood flow, pressure may rise in the lung circulation and trigger coughing or breathlessness. Some people wake needing extra pillows or needing to sit upright. A dry cough can also come from asthma, reflux, infection, allergies, or a medicine effect, so the symptom alone does not identify heart failure. Cough is a common presentation of heart failure, but many people with cough do not have it.

Note when the cough begins, whether it follows lying down, and whether you also notice breathlessness, wheeze, swelling, weight change, or fatigue. Use the sleeping position recommended by your clinician and take medicines as prescribed. Do not add cough remedies, stop a heart medicine, or change fluid intake without checking first. Arrange an assessment if this is new, persistent, or changing.

Call emergency services for severe breathing difficulty, inability to speak comfortably, chest pressure, fainting, blue-grey lips, or coughing up pink or frothy fluid. Seek prompt medical advice when a nighttime cough is rapidly worsening, repeatedly wakes you, or occurs with new swollen ankles, marked fatigue, or palpitations. These changes may indicate fluid congestion or another condition needing treatment.

Ask whether the pattern suggests fluid congestion, reflux, airway disease, infection, or a medicine effect. Discuss whether you need an examination, chest imaging, heart tests, or a chronic disease review, and ask which symptoms should trigger same-day help.
