Common signs of prostate cancer

Prostate cancer may present with difficulty starting urine, a weak or interrupted stream, straining, incomplete emptying, or needing to urinate more often, especially at night. These problems can also arise from non-cancerous prostate enlargement, infection, medicines, or bladder conditions, so their presence does not establish cancer.

Cancer that has spread beyond the prostate can present with persistent back, hip, or pelvic pain. Loss of appetite, unintended weight loss, fatigue, leg swelling, or weakness may occur in more advanced illness. None of these symptoms identifies the cause on its own; examination and appropriate tests are needed.

Arrange a medical appointment if urinary changes persist, recur, or disrupt sleep and daily activity. Note when they began, whether there is pain or blood, and which prescription and non-prescription medicines you take. Assessment may include a symptom history, examination, urine testing, and a blood test related to prostate activity, followed by imaging or biopsy when indicated.

Treatment is chosen only after the cause and, if cancer is found, its extent and features are understood. Options for prostate cancer can include monitoring, prostate removal surgery, external radiotherapy, brachytherapy, hormone-based treatment, or systemic therapy. Urinary symptoms may need separate relief even while cancer assessment continues.

Seek urgent care if you cannot pass urine, pass blood clots, develop fever with painful urination, or have severe lower abdominal or flank pain. Emergency assessment is needed for new leg weakness or numbness, difficulty walking, loss of bladder or bowel control, or numbness around the buttocks or genitals, especially when accompanied by back pain.

Ask which conditions could explain your particular symptoms, what each proposed test can clarify, and whether any medicine could be affecting urination. If prostate cancer is diagnosed, ask where it is located, whether it has spread, how urgently a decision is needed, and how each option may affect urinary, sexual, and bowel function.
