Understanding a colostomy appliance

Systems may combine the adhesive base and pouch in one unit or use separate parts. The right option depends on stoma shape, abdominal contours, stool consistency, skin sensitivity, hand movement, daily routine, and personal preference.

The stoma has no voluntary closing muscle, so stool and gas pass into the pouch as the bowel works. A skin barrier fits closely around the moist red stoma and adheres to dry skin. A good seal keeps output away from the skin without pressing or cutting into the stoma.

A colostomy may be temporary or long term, depending on the operation and the condition being treated. Stoma size and abdominal shape can change after surgery, so the opening and product choice may need reassessment during recovery.

Empty the pouch before it becomes heavy, following the instructions for your product. For a change, gently remove the adhesive while supporting the skin, clean around the stoma with warm water, dry thoroughly, measure the stoma when advised, and apply the new barrier without creases. Avoid putting oily creams beneath the adhesive unless your stoma nurse recommends a compatible product.

Check the skin and stoma each time you change the system. Repeated leakage is a fit problem rather than something to tolerate; note where the seal fails and contact a stoma nurse for adjustment. Carry spare supplies when away from home.

Contact your stoma or surgical team for persistent soreness, broken or weeping skin, frequent leaks, a new bulge, marked stoma swelling, ongoing bleeding, or a change that prevents the appliance from fitting. A small spot of blood after gentle cleaning can occur because stoma tissue is delicate, but bleeding that continues needs assessment.

Seek urgent care if the stoma becomes dark, pale, or cold; if severe abdominal pain or repeated vomiting develops; or if output stops with increasing swelling and cramps. These changes can signal impaired blood supply or bowel blockage.

Ask how closely to cut the barrier, when to remeasure the stoma, how often your specific appliance should be changed, and which accessories suit your skin and body shape. Discuss bathing, exercise, travel, clothing, gas, odour, and whom to contact if supplies fail or output changes.
