Cold hands and shortness of breath: what to know

Short answer
Cold hands with shortness of breath can reflect anxiety, airway disease, anaemia or a heart problem, and severe breathing difficulty is urgent.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- First step
- Stop exertion and sit upright
- Emergency sign
- Blue lips, collapse or throat swelling
- Review
- New or recurring breathlessness
How the symptoms can connect
Breathlessness may make the body release adrenaline, which can narrow blood vessels in the hands and cause cool, clammy skin. It can occur with asthma, acute bronchitis, COPD or bronchiectasis, especially with cough, wheeze or chest tightness. Anaemia can reduce oxygen delivery and may present with breathlessness, tiredness and paleness. Heart conditions, including angina or atrial fibrillation, can cause breathlessness with palpitations or chest discomfort. Sudden symptoms after food, medicine or an insect sting raise concern for anaphylaxis.
Consider the onset, activity, cough, wheeze, fever, swelling, chest pain, colour change and exposure to allergens or smoke. Cold hands do not show which condition is responsible; breathing severity and associated signs guide urgency.
What to do now
Stop exertion and sit upright in a position that makes breathing easier. Move away from smoke or a known irritant. Use a prescribed reliever inhaler according to your action plan. If you have a prescribed adrenaline auto-injector and symptoms suggest anaphylaxis, use it as instructed and call emergency services. Do not wait for cold hands to improve if breathing is getting harder.
When to seek emergency help
Call emergency services for severe or rapidly worsening breathlessness, inability to speak comfortably, blue or grey lips, confusion, collapse, severe chest pain, coughing blood, or swelling of the tongue or throat. Obtain urgent assessment for new breathlessness, symptoms at rest, faintness, or a cold pale hand with pain. Arrange a clinical review for recurring breathlessness, reduced exercise tolerance or symptoms that wake you at night.
Questions for your doctor
Ask whether you need oxygen measurement, lung testing, an ECG, chest assessment or blood tests for anaemia. Explain your usual activity level, smoking or irritant exposure, allergies, medicines, inhaler use and the exact sequence of symptoms. Confirm your personal emergency plan.

Questions people ask
Can anxiety cause shortness of breath and cold hands?
Anxiety can cause rapid breathing and adrenaline-related cool, clammy hands. Because breathlessness has other causes, new or severe symptoms need medical assessment.
When should I call emergency services?
Call for severe or worsening breathing difficulty, blue or grey lips, collapse, confusion, severe chest pain, or tongue or throat swelling.
