What should you know about cold feet, fever and headache?

The combination does not identify one diagnosis by itself. Fever can narrow blood vessels in the skin while the body prioritises blood flow to internal organs, making hands or feet feel cool. Chills can also make the feet feel cold even when the measured temperature is raised.

What these symptoms may mean

A viral illness such as a common cold or COVID-19 can cause fever and headache, sometimes alongside chills, tiredness or body aches. Dehydration from fever, sweating, vomiting or reduced drinking can add headache and cold-feeling extremities. Heat exhaustion can also cause headache and weakness, especially after heat exposure.

Sinus inflammation may contribute to a blocked nose, facial pressure and headache. Anxiety can temporarily alter breathing and blood flow to the hands and feet, but fever should prompt consideration of a physical illness as well. Dengue fever is another condition that needs medical assessment when fever is accompanied by severe headache, marked aches, rash, nausea or unusual bleeding, particularly after possible mosquito exposure.

What to do now

Rest in a comfortable room, measure your temperature with a thermometer and record when the symptoms began. Sip water or an oral rehydration drink if you have been sweating or have had stomach symptoms. Wear light, warm socks, but avoid very hot water or direct heat on numb feet.

Follow the label directions for any fever or headache medicine and check with a pharmacist if you have a long-term condition, take other medicines, or may have dengue. Avoid strenuous activity while feverish. If symptoms are worsening, lasting longer than expected, or you cannot keep fluids down, arrange a clinical assessment.

When to seek medical care

Seek urgent medical help for confusion, fainting, trouble breathing, a stiff neck, a sudden severe headache, a seizure, blue or very pale skin, or a foot that becomes painful, numb or noticeably discoloured. Get prompt advice for repeated vomiting, very little urine, a spreading rash, unusual bleeding, severe abdominal pain or rapid deterioration.

Arrange same-day medical advice if the fever is high or persistent, the headache is severe or new, one foot is much colder than the other, or you have diabetes, poor circulation, reduced immunity, or are pregnant. A clinician may check hydration, circulation, temperature and signs of infection, then decide whether testing is needed.
