Cold, cough and blood in phlegm: what to know

Frequent or forceful coughing can irritate small blood vessels in the nose, throat, or airways, leaving pink or rust-coloured streaks in mucus. A common cold, acute bronchitis, or allergic rhinitis may produce this pattern. Asthma can cause cough and tightness, while bronchiectasis may cause recurring mucus and bleeding. Bronchiolitis is mainly a concern in young children. Acid reflux can trigger a persistent cough but does not explain every episode of blood. The appearance and amount of blood, associated symptoms, smoking history, and duration all matter.

Arrange medical advice rather than ignoring blood in phlegm, especially if it returns. Note whether the blood is streaked through mucus or separate, its colour and approximate amount, and whether it follows a nosebleed or forceful cough. Rest, drink fluids, avoid smoking and vaping, and do not take cough suppressants or blood-thinning medicines differently from prescribed without advice. A clinician may listen to your chest and decide whether testing is needed.

Call emergency services for more than a small amount of blood, bleeding that does not stop, severe breathlessness, chest pain, fainting, blue lips, confusion, or a very fast heartbeat. Seek prompt review for repeated blood-streaked mucus, fever, worsening cough, unexplained weight loss, night sweats, or a cough that persists. Children, pregnant people, and anyone taking anticoagulants should seek advice early.

Ask whether the blood appears to come from the nose, throat, or lungs, whether you need a chest examination or imaging, and how to manage your medicines while bleeding is being assessed.
