The main distinction

Clinical illness may include a painful, warm or reddened breast area together with fever, chills or a flu-like feeling. The term subclinical is used when milk or inflammatory findings suggest a problem despite few obvious symptoms; it is not something a parent can confirm by touch alone.

What happens in the breast

Mastitis sits on a spectrum of inflammation. Milk stasis, tissue swelling and pressure can narrow milk flow, and bacterial infection may develop in some cases. Nipple trauma, an oversupply, abrupt changes in feeding or pumping, and sustained pressure on the breast can contribute.

Not every tender area is bacterial mastitis, and antibiotics are not automatically required. Conversely, feeling very unwell with breast changes merits clinical assessment even when redness is difficult to see on darker skin.

Supporting recovery and feeding

Continue breastfeeding responsively if you are able, without repeatedly trying to empty the breast. Use a comfortable position, avoid deep massage, and apply a wrapped cold pack for swelling. Rest, drink according to thirst and consider suitable pain relief after checking what is safe for you.

A feeding specialist can assess attachment, nipple damage, milk supply and pump fit. If a clinician prescribes antibiotics, take them as directed and complete the course. Sudden weaning can increase milk stasis, so seek a gradual plan if you want to stop feeding.

Red flags and follow-up

Seek prompt assessment for fever, chills, spreading breast redness, worsening pain, pus, or a soft or fluctuant swelling. Also get reviewed if symptoms are not beginning to improve with supportive care or if they return after treatment.

Urgent medical help is needed for confusion, fainting, severe weakness, breathing difficulty or inability to keep fluids down. A persistent lump after inflammation settles requires examination to exclude an abscess or another breast condition.

Ask whether your presentation is inflammatory or bacterial, whether an abscess needs imaging, how to adjust feeding or pumping, which pain relief suits you, and when persistent or recurrent symptoms need milk testing or specialist referral.

No. Mastitis can begin as inflammation without bacterial infection. A clinician uses the symptom pattern and examination to decide whether antibiotics are appropriate.

Can I breastfeed when I have clinical mastitis?

How would I know I have subclinical mastitis?
