Why exercise selection matters

A sound without pain during smooth movement may not need to be eliminated. Exercise is more likely to need modification when the click coincides with a pinch, sudden catch, loss of control, or apprehension. For some people with shoulder instability, the joint feels as though it slips; repeatedly loading that position without guidance may aggravate symptoms.

Mobility work addresses restricted movement, while control and resistance exercises train muscles to guide the shoulder and blade. An assessment helps decide which limitation is actually present.

Safer training principles

Begin with an unloaded, comfortable range and slow tempo. A gentle shoulder-blade set, supported arm raise, or outward rotation with the elbow kept near the side may be suitable examples when each movement stays controlled and does not increase symptoms.

Stop a drill that produces sharp pain, repeated catching, numbness, or a sense of slipping. Avoid forcing deeper range to chase a click. Progress resistance or overhead height only when the current movement remains comfortable during the session and everyday activity is not worse afterward.

When exercise should wait

Get assessed before continuing shoulder exercise if clicking followed a fall, dislocation, or sudden pull; if strength has dropped; or if the shoulder locks or repeatedly gives way. Persistent night pain, swelling, warmth, or steadily shrinking range also needs clinical review.

Seek urgent care for deformity after injury, inability to lift the arm, severe worsening pain, or new marked numbness, weakness, coldness, or colour change in the hand.
