Classic LCIS: what it means and what comes next

LCIS stands for lobular carcinoma in situ. The cells remain contained in the milk-producing lobules, so classic LCIS is not invasive breast cancer. Its presence matters because later cancer can arise in either breast, not only where LCIS was found.

Classic LCIS is often discovered unexpectedly in breast tissue removed for another reason. Under the microscope, the lobules are expanded by small, fairly uniform cells. Pathologists may use E-cadherin staining to support lobular differentiation when the appearance is uncertain.

The word carcinoma in the name can be alarming, yet classic LCIS behaves as a risk indicator and a possible non-obligate precursor rather than an invasive tumour. Florid or pleomorphic LCIS has different microscopic features and may lead to a different management discussion, so the exact wording of the report is worth confirming.

A breast specialist usually checks that imaging, the reason for the biopsy, and the pathology agree. If classic LCIS was found on a needle biopsy, the team considers whether another area needs sampling or removal based on any imaging–pathology mismatch or additional abnormality. Ongoing care may involve regular clinical review and breast imaging. Risk-reducing endocrine therapy may be offered after weighing expected benefit against side effects, medical history, menopause status, and personal preferences. Removing both breasts is rarely the default response to classic LCIS alone.

Classic LCIS itself generally does not produce a breast lump or a visible change. Arrange a medical assessment for a new lump, nipple discharge that is bloody or clear, nipple inversion that is new, skin dimpling, persistent focal breast pain, redness that does not settle, or a change in breast shape. These findings warrant evaluation because of the change itself; they do not establish a cancer diagnosis.

Ask whether the report specifically says classic LCIS, whether the imaging and pathology are concordant, and whether the finding was completely represented by the biopsy. Also discuss the imaging schedule, your personal and family history, whether genetic counselling is relevant, and the advantages and side effects of medication for risk reduction.
