How this condition affects the body

Some blood clots in the lungs do not clear fully. Scar-like material can narrow the pulmonary arteries, making it harder for blood to pass through the lungs. The right side of the heart then works against greater resistance. Breathlessness on exertion, tiredness, chest discomfort, dizziness, or ankle swelling may develop gradually. Similar symptoms occur with heart failure, anaemia, obesity, sleep apnoea, and other conditions, so symptoms alone cannot establish the diagnosis. Assessment may include heart and lung examination, imaging of the pulmonary arteries, heart ultrasound, breathing tests, and measurement of pressures by a specialist team.

Managing the condition

Attend specialist appointments and take anticoagulant or other prescribed treatment exactly as directed. Tell the team about bleeding, new medicines, pregnancy plans, and any change in exercise tolerance. Activity should be increased only within an individual plan; stop and seek advice if exertion causes marked dizziness, chest pain, or severe breathlessness. Treating related problems such as high blood pressure, sleep apnoea, or excess weight may support overall cardiovascular health, but these steps do not replace specialist treatment for the narrowed lung arteries. Keep a list of symptoms and questions for each review.

When symptoms are urgent

Call emergency services for sudden severe breathlessness, chest pain, fainting, coughing blood, new confusion, or blue lips. Seek prompt advice for rapidly worsening exercise tolerance, new palpitations, or increasing ankle swelling. If you take an anticoagulant, urgent review is needed for heavy bleeding, black stools, vomiting blood, a serious head injury, or a severe unexplained headache. Do not stop anticoagulation without medical advice unless emergency clinicians instruct you.
