Chronic hepatitis B and life expectancy

A clinician cannot estimate one person's life expectancy from a positive hepatitis B result alone. Blood tests, liver imaging, medical history, and signs of liver scarring provide a more useful picture.

Chronic infection can remain quiet or cause ongoing liver inflammation. Over time, persistent inflammation may lead to fibrosis, cirrhosis, liver failure, or liver cancer. Risk differs between people and can change as viral activity and liver condition change.

Doctors assess hepatitis B DNA, liver enzymes, hepatitis B antigens, platelet count, and liver stiffness or imaging findings. Other liver stressors, including alcohol, fatty liver disease, hepatitis C or D, and some medicines or supplements, can also affect long-term health.

Keep scheduled blood tests and scans even when you feel well, because liver damage may develop without obvious symptoms. If antiviral treatment is prescribed, take it consistently and do not stop it without medical guidance; stopping can allow the virus to become active again.

Avoid alcohol, check medicines and herbal products with a clinician or pharmacist, and address diabetes or excess liver fat if relevant. Household and sexual contacts should ask whether they need testing and hepatitis B vaccination.

Arrange a prompt review for new yellowing of the skin or eyes, dark urine, increasing abdominal swelling, persistent vomiting, unusual bruising, or worsening fatigue. Seek emergency care for confusion, severe drowsiness, vomiting blood, black stools, or sudden severe abdominal pain. These signs can indicate significant liver dysfunction or bleeding and should not wait for a routine appointment.

Ask what your latest hepatitis B DNA and liver enzyme results mean, whether there is evidence of fibrosis, and whether antiviral treatment is appropriate now.
