What postpartum itching may mean

Intrahepatic cholestasis of pregnancy affects bile flow and can cause intense itching, often on the palms or soles, without a primary rash. After birth, hormone changes and recovery of bile handling may not make the sensation disappear immediately. A clinician may review your symptoms and arrange blood tests that assess liver function and bile acids. New bumps, scaling, redness, or a visible rash can point toward a separate skin problem such as Atopic Dermatitis, Contact Dermatitis, or Hives.

Practical steps after birth

Tell your maternity team or doctor how strong the itch is, where it occurs, whether sleep is affected, and whether it is improving. Keep nails short, use cool compresses, choose loose cotton clothing, and use an unperfumed moisturiser. Avoid starting prescription, herbal, or over-the-counter medicines while breastfeeding without checking suitability. Keep any follow-up appointments so symptoms and liver-related tests can be reviewed together.

Contact your maternity service promptly if itching remains intense, returns after improving, or comes with yellowing of the eyes or skin, dark urine, pale stools, nausea, or pain under the right ribs. Seek urgent help for severe illness, confusion, fainting, breathing difficulty, or heavy postpartum bleeding. These warning signs require assessment and should not be explained away as ordinary recovery.
