Childhood alopecia areata: what families should know

The scalp may show one or more smooth patches without obvious scaling or scarring. Some children also develop changes in the nails or loss of eyebrow and eyelash hair. The course can be unpredictable: patches may regrow, recur or become more extensive, so a single photograph cannot predict what will happen next.

Alopecia Areata is not caused by poor hygiene and is not contagious. Hair loss with scale, broken hairs, crusting or tenderness may point to another condition, including Scalp Ringworm, which needs different treatment.

Arrange a medical assessment rather than pulling at hairs or applying adult products. Gentle hair care, sun protection on exposed scalp and comfortable hats can protect bare areas. Listen for teasing, anxiety or reluctance to attend school; emotional support is part of practical care.

Treatment depends on the child’s age, the extent and location of loss, and how active it appears. Options a specialist may discuss include Topical Corticosteroids, Steroid Injection Into The Skin for selected situations, or JAK Inhibitor Treatment. Oral Corticosteroid Treatment may be considered only in particular circumstances.

Seek assessment promptly when bald patches appear, enlarge quickly, involve brows or lashes, or occur with scalp inflammation. Ask for earlier review if the child is distressed, being bullied, or avoiding normal activities. Urgent care is appropriate for severe swelling, infection-like drainage or significant illness alongside the scalp changes.

Ask how the diagnosis was reached, whether the nails or other skin need checking, and what signs would suggest Scalp Ringworm instead. Discuss realistic treatment goals, side effects, school support and how often the scalp should be reviewed.
