CHF and shortness of breath: what should you know?

Breathlessness from heart failure may begin with exertion, then occur during ordinary activities or when lying flat. Waking suddenly short of breath, needing extra pillows, a new cough, ankle swelling, or reduced stamina can occur with fluid congestion. The symptom can have many other explanations, including [Asthma](/conditions/asthma/), [COPD](/conditions/copd/), anaemia, infection, angina, or a heart-rhythm problem such as [Atrial Fibrillation](/conditions/atrial-fibrillation/).

Stop and rest upright, loosen restrictive clothing, and use any prescribed rescue plan. Take heart medicines as directed and do not double a missed dose unless a clinician tells you to. Note what you were doing, whether lying down worsened it, how long it lasted, and any swelling, cough, chest discomfort, or palpitations. New or steadily worsening breathlessness deserves prompt clinical assessment even when it settles with rest.

Call emergency services for severe breathlessness at rest, blue or grey lips, chest pressure, fainting, confusion, coughing pink or frothy fluid, or inability to speak in full sentences. Seek urgent advice for breathlessness that is rapidly worsening, repeatedly wakes you, or comes with a new irregular heartbeat or marked weakness.

Ask whether your pattern suggests fluid congestion, lung disease, anaemia, or a rhythm issue; which symptoms require emergency help; how to use your medicines during an illness; and whether your treatment or fluid plan needs review.
