Chest tightness and heartburn: what to know

Short answer
Heartburn can cause burning chest discomfort, but chest tightness may have other causes and severe unfamiliar symptoms need urgent assessment.

Heartburn and chest tightness
Heartburn happens when stomach contents move back towards the food pipe, causing burning behind the breastbone, a sour taste or irritation in the throat. Acid Reflux may worsen after a large meal, when bending or when lying down. Chest tightness can also occur with Asthma, COPD, muscle strain, anxiety or Angina, so relief after an antacid does not by itself prove the cause.
Describe the sensation precisely: burning versus pressure, its relation to meals and posture, and whether there is breathlessness, sweating, nausea, dizziness or pain spreading beyond the chest. New exertional discomfort deserves medical review even when heartburn is familiar.
Helpful measures
Remain upright after eating, choose smaller meals, and avoid personal triggers such as late meals, alcohol or foods that reliably worsen symptoms. Ask a pharmacist or clinician about suitable reflux treatment, particularly if symptoms recur or swallowing becomes difficult. Do not use self-treatment to delay care for unusual chest pressure.
When to get urgent help
Call emergency services for severe, persistent or exertional chest pressure, or discomfort with breathlessness, fainting, cold sweating, marked weakness, nausea, or spread to the arm, jaw, back or neck. Seek prompt assessment for vomiting blood, black stools, food sticking, unexplained weight loss or rapidly worsening symptoms.
Questions for your doctor
Does the timing with meals and posture fit reflux, or should my heart and lungs be checked? Which reflux treatments are appropriate for me? What changes in swallowing, breathing or chest discomfort should prompt urgent help?

Can heartburn feel like chest tightness?
Reflux may cause burning or pressure-like discomfort behind the breastbone, but similar sensations can have heart or lung causes.
Should I assume chest discomfort is heartburn?
No. New, severe, exertional or unfamiliar discomfort needs medical assessment, especially with breathing difficulty, sweating, faintness or spreading pain.
