Why cold air may matter

Cold, dry air can irritate the airways and make breathing feel tight, especially during exercise or in people with sensitive airways. Cold exposure also makes the body work harder to conserve heat; in someone with narrowed heart arteries, exertion in the cold can bring on Angina. The temperature may reveal a pattern rather than be the whole cause.

Chest symptoms can arise without a direct cold-weather mechanism. Acid Reflux or Gastritis may cause burning, while Pneumonia can follow an infection with cough and fever. Pulmonary Embolism, Panic Disorder, and Shingles are other possible explanations with distinct warning features.

Steps that may help

Move indoors, rest, and warm the chest and face gradually. Covering the mouth and nose with a loose scarf can warm inhaled air; avoid strenuous activity until unexplained pain has settled. If cold repeatedly triggers symptoms, record the temperature, activity, pain quality, breathing changes, cough, and recovery time.

Seek a clinical review for recurring episodes, pain during walking or exercise, or breathlessness that is new. Do not test the pattern by deliberately exercising in cold conditions. Use prescribed medicines only as directed and do not borrow an inhaler or heart medicine.

Urgent symptoms

Call emergency services for chest pressure that is severe, persistent, or spreading to the arm, jaw, back, or neck; major breathing difficulty; fainting; confusion; blue lips; cold sweating; or sudden weakness. Coughing blood, a fast heartbeat, or one-sided leg swelling with chest pain also needs immediate help.
