Possible explanations

Tightness may feel like squeezing, pressure or difficulty taking a full breath. Asthma and COPD can narrow or inflame the airways, while mucus can make the chest feel blocked. Acid reflux may cause burning or pressure behind the breastbone. Anxiety and panic can produce chest sensations with rapid breathing, trembling or a racing heart. Angina is a heart-related cause of chest discomfort, often brought on by exertion. Symptoms overlap, so the sensation alone cannot identify the cause.

What to do now

Stop strenuous activity and sit upright while you assess how you feel. Move away from smoke or other irritants. If a clinician has prescribed an inhaler plan, follow that plan exactly; do not borrow inhalers. Record whether symptoms follow exercise, meals, stress or a particular exposure. A GP can check your heart and lungs, review medicines and decide whether tests are needed.

Urgent warning signs

Call emergency services for severe or sudden chest pressure, trouble speaking because of breathlessness, fainting, blue lips, new weakness, sweating with nausea, or pain spreading to the arm, jaw or back. Seek same-day advice for new tightness, symptoms triggered by activity, or a change in your usual asthma or COPD pattern. Do not drive yourself if you feel faint or severely unwell.

Ask whether your symptoms fit an airway, reflux, anxiety or heart pattern; whether you need an ECG, breathing assessment or other test; and which warning signs should bring you back. Tell the clinician exactly where the discomfort is, what brings it on, how long it lasts and whether it changes with breathing or movement.
