A cramp usually feels like the back of the thigh suddenly tightening into a hard knot. A muscle contusion is more likely after an impact and may cause tenderness, swelling, bruising, and pain when walking or bending the knee.
Cramp, bruise, or strain?
The event that came before the pain provides a useful clue. Tightening during activity or at rest without an impact fits a cramp. After a blow, use a wrapped cold pack, rest from painful activity, and elevate the leg if swollen. Avoid forceful stretching, deep massage, or training through sharp pain while the area is newly bruised or swollen. Resume movement gradually when walking and bending the knee are comfortable. A physiotherapist can assess persistent pain and guide mobility, strength, and return to sport.
When to seek medical care
Arrange prompt assessment if pain is severe, swelling or bruising is expanding, you cannot bear weight, the knee will not bend normally, or symptoms followed a pop or major impact. Repeated cramps, ongoing weakness, or pain that does not steadily settle also deserves evaluation.
Seek urgent help if the leg becomes cold, pale, numb, or increasingly tight, or if thigh swelling occurs with chest pain, faintness, or shortness of breath. These features need immediate assessment rather than home treatment.
