Pelvic or cervix pain with no period

A person cannot locate pelvic pain precisely by feel, so discomfort described as cervical may come from the uterus, ovaries, bladder, bowel, pelvic floor or cervix. A missed period and pelvic pain need to be considered together rather than assumed to have one cause.

Pregnancy is an early possibility to check. An ectopic pregnancy can present with a missed period and pelvic pain and requires urgent assessment. Polycystic ovary syndrome can present with irregular or absent periods, while menopause and premature ovarian insufficiency involve changes in ovarian hormone activity and menstrual patterns. Cysts, infection, endometriosis, urinary conditions, bowel problems and pelvic floor tension may explain pain without being the reason a period is absent.

Take a home pregnancy test if pregnancy could be possible, following the kit instructions. Arrange a medical appointment if the result is negative but periods remain absent, or if pain returns or continues. Note the date of your last period, contraception, recent pregnancy tests, pain location, vaginal bleeding or discharge, and urinary or bowel changes. A clinician may select examination, blood tests, infection tests or pelvic imaging based on this information.

Get urgent medical help for severe or rapidly worsening pelvic pain, fainting, marked dizziness, shoulder-tip pain, heavy vaginal bleeding, breathing difficulty, fever or repeated vomiting. If pregnancy is possible, seek prompt assessment for one-sided pain or bleeding even when a test is negative or has not yet been taken. New unusual discharge, pain when urinating, or difficulty passing urine or stool also calls for timely evaluation.

Ask which causes fit both the menstrual change and the pain, whether pregnancy has been excluded adequately, and which tests are relevant now. Mention medicines, hormonal contraception, breastfeeding, major stress, weight change and symptoms such as hot flushes, acne or increased facial hair, because these details can guide the assessment.
