Deep pain during sex around ovulation

You cannot reliably identify the cervix as the source by sensation alone. Deep penetration may press on the cervix, uterus, pelvic floor, ovaries or other tender tissue, while cycle timing can make an ovarian source more noticeable.

Ovulation can cause brief, one-sided lower abdominal discomfort, and deep penetration may intensify tenderness at that time. Endometriosis can present with deep pain during or after sex and pain that follows a cycle pattern. Pelvic inflammatory disease can present with deep pain during sex alongside pelvic pain, unusual discharge or bleeding. Ovarian cysts, pelvic floor muscle tension, vaginal dryness and irritation can also contribute. The timing is a clue, not a diagnosis.

Pause any activity that hurts and try shallower penetration, a different position and adequate lubricant if you want to continue. Record where the pain is felt, whether it occurs on one side, its relationship to the cycle, and any discharge, bleeding, bowel or urinary symptoms. Arrange a gynaecology or primary-care review when pain repeats or affects intimacy. Assessment may include a pregnancy test, infection testing, pelvic examination or ultrasound, chosen according to your symptoms.

Seek urgent medical assessment for sudden severe or worsening pelvic pain, faintness, dizziness, shoulder-tip pain, heavy bleeding, fever, vomiting, or difficulty breathing. Urgent review is also appropriate if pregnancy is possible, even without a positive test, because pelvic pain can accompany an ectopic pregnancy. New pain with unusual discharge, bleeding between periods or pain when urinating needs prompt evaluation for infection or another pelvic condition.

Describe whether the pain is at the vaginal entrance or deeper in the pelvis, whether it continues after sex, and how closely it tracks ovulation. Ask whether infection testing, examination or imaging would help, how to manage pain while the cause is investigated, and whether pelvic floor physiotherapy is appropriate.
