Cellulitis and Osteomyelitis: Understanding Skin and Bone Infection

Short answer
Cellulitis infects deeper skin tissues, while osteomyelitis involves bone and usually needs more extensive investigation and treatment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Cellulitis
- An infection involving deeper layers of skin and underlying soft tissue.
- Osteomyelitis
- An infection within bone that can require targeted antibiotics and surgery.
- Do not delay
- Rapidly spreading redness or systemic illness needs urgent assessment.
The key distinction
The conditions can occur separately. A skin infection may occasionally spread into nearby bone, particularly when there is a deep wound, ulcer, recent operation, poor circulation, or implanted hardware, but cellulitis does not automatically mean bone infection.
What each infection can look like
Cellulitis commonly presents with an expanding area of red or darker-than-usual skin that feels warm, swollen, and tender. Osteomyelitis may present with focused deep bone pain, swelling, warmth, drainage from a wound, fever, or reduced use of the affected limb. Symptoms can be subtle when infection has persisted or when circulation or sensation is impaired.
A clinician examines the skin, wounds, circulation, and nearby joints. Blood tests can look for inflammation and infection. Imaging may be needed to assess bone, while a sample of blood, wound material, or bone can help identify the organism and guide antibiotic selection.
What to do if infection is suspected
Obtain same-day medical advice for spreading redness, increasing warmth, worsening swelling, discharge, or fever. Marking the edge of redness and noting the time may help show whether it is expanding, but this should not delay care. Keep any wound clean and covered, avoid squeezing it, and do not use leftover antibiotics.
Treatment depends on depth and severity. Cellulitis is generally treated with an appropriate antibiotic. Osteomyelitis often requires a longer, targeted antibiotic course and sometimes surgery to drain infection or remove unhealthy tissue. Complete medicine exactly as prescribed and attend wound and laboratory follow-up.
Warning signs that need urgent care
Go for urgent assessment if redness spreads quickly, pain becomes severe or seems out of proportion, skin develops blisters or dark patches, or fever is accompanied by confusion, faintness, rapid breathing, or profound weakness. These features can accompany a rapidly progressing infection.
Prompt assessment is also needed when infection surrounds an operation site or implant, lies over an exposed bone, follows a puncture wound, or occurs with diabetes, impaired circulation, or reduced immune function. New numbness, a cold limb, or inability to move a nearby joint also warrants urgent care.
Questions that clarify the plan
Ask whether the findings are limited to skin or suggest bone involvement, what sampling or imaging is needed, and when improvement should become noticeable. Clarify how the wound should be dressed, whether weight-bearing needs to change, which medicine effects need attention, and who will review the response.

Questions people ask
Can cellulitis turn into osteomyelitis?
A skin infection can sometimes extend into nearby bone, especially through a deep wound or ulcer. Many cases remain limited to soft tissue, so examination and appropriate tests determine the depth.
How is osteomyelitis confirmed?
Clinicians combine examination, blood tests, and imaging. A blood, wound, or bone sample may be used to identify the organism and select a suitable antibiotic.
Is surgery always needed for a bone infection?
No. The decision depends on the infection site, damaged tissue, abscesses, blood supply, implants, and response to antibiotics. Some cases need drainage or removal of unhealthy bone.
