CDK4 inhibitors in breast cancer care

They are commonly paired with endocrine treatment rather than used as a substitute for every other therapy. The exact medicine and treatment setting depend on tumour features, previous care and the person's health.

CDK4 and CDK6 are proteins involved in moving cells through the division cycle. Blocking them can hold susceptible cancer cells in a resting phase. The term usually refers to medicines such as palbociclib, ribociclib or abemaciclib, which have different schedules, interactions and side-effect patterns.

Eligibility is based mainly on the cancer's hormone-receptor and HER2 status, its stage, earlier treatments and clinical circumstances. This is different from inheriting a CDK4 gene variant, and it does not mean that everyone with breast cancer needs genomic tumour profiling.

Before starting, give the oncology team a full list of prescriptions, supplements and herbal products because interactions can change drug levels. Monitoring may include blood counts, liver tests and, for some medicines, heart-rhythm checks. Follow the prescribed schedule and contact the team for instructions after a missed dose rather than doubling it.

Discuss diarrhoea prevention and treatment if abemaciclib is planned. Infection precautions may matter when white blood cells fall. Pregnancy should be avoided during treatment, and contraception or fertility questions should be raised before the first dose.

Call the oncology team promptly for fever, chills, persistent diarrhoea, inability to drink, unusual bruising, yellowing of the skin or eyes, a new rash, or worsening exhaustion. Seek urgent care for trouble breathing, chest pain, fainting, confusion, or one-sided leg swelling and pain. Use the team's temperature threshold and emergency instructions because low white-cell counts can make infection serious.
