CCCA Symptoms: What to Know

Central centrifugal cicatricial alopecia often presents with gradual thinning at the crown that enlarges outward. The scalp may feel itchy, sore, warm, or burning, and some people see scaling, pimples, or short broken hairs. Others have little discomfort and notice the change while styling or parting the hair.

These symptoms are not unique to CCCA. Irritation, infection, traction, and non-scarring hair-loss conditions can look similar at first. A dermatologist considers the distribution of loss, the appearance of follicle openings, the scalp surface, and the history of styling or chemical exposure.

Arrange a scalp examination if symptoms persist, recur, or accompany widening crown loss. Take photographs and note when itching, pain, or shedding began. Keep hairstyles loose and avoid heat, scratching, and products that sting until the cause is clearer.

A clinician may recommend prescription anti-inflammatory treatment, including medicine applied to the scalp, injections into selected areas, or an oral option. Use treatment exactly as directed and report irritation or other new symptoms rather than adding products yourself.

Seek prompt care for rapidly spreading loss, severe pain, pus, open sores, thick crusts, marked swelling, or fever. Such changes can indicate infection or intense inflammation. Sudden widespread shedding also deserves evaluation, even when the crown was the first area you noticed.

Ask which symptoms suggest active follicle inflammation, whether a biopsy would clarify the diagnosis, and how often follow-up is needed. Ask for a styling plan that avoids traction and for clear instructions about prescribed scalp treatment.
