CBT for depression: what to know

How the structures involved in Depression differ from normal

Short answer

CBT for depression is a structured talking therapy that helps you notice unhelpful thought and behaviour patterns, practise alternative responses, and build routines that support recovery.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What CBT means

Cognitive behavioural therapy links thoughts, feelings, physical sensations, and actions. Depression can narrow attention towards criticism, hopeless predictions, or withdrawal. In CBT, you and your therapist examine these patterns without treating every thought as a fact. You may then test a more balanced interpretation or take a small action that fits your goals.

Sessions are usually collaborative and practical. You might review your mood, identify a difficult situation, learn a skill, and agree on practice between appointments. The work can include activity planning, problem-solving, managing self-critical thoughts, and rebuilding rewarding or meaningful activities. Treatment may be offered individually, in a group, or through a guided digital programme, depending on your needs and local services.

CBT does not require you to pretend that difficult experiences are positive. It aims to help you respond to them more flexibly. Depression may also involve fatigue, difficulty concentrating, sleep changes, or feeling anxious; these concerns can be included in the treatment plan.

How to get started

Arrange an assessment with a qualified mental health professional or a primary-care clinician. They can discuss your symptoms, daily functioning, previous treatment, medicines, sleep, substance use, and any safety concerns before suggesting CBT or another approach. CBT can be used on its own or alongside other depression treatments when clinically appropriate.

Before the first appointment, write down changes in mood, sleep, energy, concentration, appetite, and activities. Bring a list of medicines and supplements, and mention conditions such as insomnia or anxiety. Ask how sessions are delivered, what between-session practice involves, how progress will be reviewed, and what to do if your mood worsens.

Between sessions, use only the exercises you have agreed with your therapist. A brief activity plan can make goals more manageable: choose one necessary task, one supportive contact, and one activity that feels meaningful or mildly enjoyable. Record what happened rather than judging yourself if the plan was difficult.

When to seek care

Seek prompt medical advice if low mood, loss of interest, sleep disruption, fatigue, or difficulty concentrating is interfering with work, study, relationships, self-care, or safety. A clinician can check for depression and other causes of similar symptoms and help match the level of support to your situation.

If you are thinking about suicide, self-harm, or harming someone else, or you cannot keep yourself safe, treat this as an emergency. Contact local emergency services or go to the nearest emergency department now. If possible, tell a trusted person, stay with them, and move away from anything you could use to hurt yourself.

Contact your therapist or clinician promptly if distress rises sharply, you cannot manage agreed activities, or treatment feels overwhelming. Do not stop prescribed medicines or change their dose without medical advice.

Questions for your doctor or therapist

Ask: Is CBT suitable for my symptoms and circumstances? What format and session schedule do you recommend? How will we measure progress? What should I do if exercises increase distress? Could sleep problems, anxiety, medicines, or another health condition be affecting my mood? What support is available between appointments, and what is the plan if CBT is not enough?

Questions people ask

Does CBT mean that depression is your fault?

No. Depression has many possible influences, and CBT does not blame you. It focuses on patterns you can explore and skills that may make daily life more manageable.

Can CBT be used with antidepressants?

It may be used alongside prescribed treatment when a clinician considers that combination appropriate. Discuss benefits, concerns, side effects, and any medicine changes with your prescriber.

What if talking about depression feels too difficult?

Tell the therapist at the start. They can slow the pace, use written prompts, focus on immediate coping, and revisit sensitive topics when you feel more prepared.
