Past pelvic infection is an important cause. Other possibilities include endometriosis, scar tissue after pelvic or abdominal surgery, or inflammation near the tube. Only one side may be affected because the original infection, inflammation, or scarring was localised.

A hydrosalpinx is a fallopian tube whose far end is sealed and whose inner channel has become distended with fluid. Pelvic inflammatory disease can leave this type of tubal scarring, sometimes long after the original infection has settled.

The condition may cause no symptoms, or it may accompany pelvic discomfort, unusual discharge, or difficulty becoming pregnant. A scan can suggest the diagnosis, but the clinician also considers infection history, previous surgery, endometriosis symptoms, and whether the other tube appears open.

Arrange a gynaecology review to confirm the finding and discuss what matters to you, especially pain control and pregnancy plans. Assessment may include a pelvic ultrasound, infection testing, and a test of tubal openness when fertility is being investigated.

Management depends on symptoms, the condition of the other tube, and reproductive goals. Observation may suit an incidental finding without troublesome symptoms. When treatment is needed, options can include salpingectomy or sealing the affected tube; IVF may be discussed when damaged tubes are affecting fertility. Antibiotics treat an active infection but do not reopen established scar tissue.

Seek prompt medical assessment for new pelvic pain, fever, foul-smelling discharge, pain during sex, or bleeding outside your usual pattern. These features can accompany an active pelvic infection and should not wait for a routine fertility appointment.

Get urgent help for severe or rapidly worsening one-sided pain, faintness, shoulder-tip pain, heavy bleeding, or pelvic pain with a positive pregnancy test or missed period. Those symptoms require assessment for pregnancy-related emergencies and other acute pelvic conditions.
