Causes of stiff neck and headache

Short answer
A stiff neck with headache can come from muscle tension, dehydration, a viral illness, sinus symptoms, or heat-related illness.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

- Headache with fever and pronounced neck stiffness needs urgent medical assessment.
Possible explanations
Poor posture, prolonged screen use, an awkward sleeping position, or stress can tighten neck and shoulder muscles and produce a pressure-like headache. Dehydration may cause headache with thirst, dark urine, dizziness, or tiredness. A common cold, COVID-19, or sinus inflammation may add facial pressure, congestion, fever, or body aches.
Heat exhaustion can cause headache, weakness, sweating, nausea, and muscle cramps after time in hot conditions. Dengue fever can present with fever and severe headache, sometimes with pain behind the eyes, body aches, nausea, or a rash; local medical advice matters if these symptoms follow mosquito exposure. Generalised anxiety disorder may contribute to muscle tension, but a new or severe headache should not be attributed to anxiety without assessment.
What you can do
Move gently, take breaks from screens, and use a comfortable neutral neck position. Drink fluids and rest in a cool place if heat or dehydration may be involved. Note the onset, severity, fever, recent illness, injury, medicines, and associated symptoms. Avoid forceful neck manipulation when the cause is unclear.
When to seek care
Seek urgent help for a sudden severe headache, marked neck stiffness with fever, confusion, fainting, a seizure, new weakness, trouble speaking, or a new rash. Prompt assessment is also needed after a head or neck injury, or when vomiting and worsening pain prevent fluids from staying down.
Questions for your doctor
Could muscle tension explain the pattern, or do I need tests for infection or dehydration? Which symptoms should prompt emergency care? How should I manage neck movement and fluids while recovering? Could my medicines or recent heat exposure be contributing?

Questions people ask
Can dehydration cause a stiff neck and headache?
Dehydration can cause headache and tiredness, while muscle tension may make the neck feel stiff at the same time.
Is a stiff neck with headache always an infection?
No. Muscle tension, heat, dehydration, sinus symptoms, and viral illnesses can all contribute, but severe or rapidly worsening symptoms need assessment.
