Causes of pre-eclampsia after delivery

Delivery removes the placenta, which is central to pre-eclampsia, but its effects on blood pressure, blood vessels, fluid balance, kidneys, liver, brain, and blood clotting can continue. Some people had raised blood pressure during pregnancy; others develop signs only after leaving the birth unit. It can follow a vaginal or caesarean birth.

Risk may be higher with previous pre-eclampsia, chronic high blood pressure, kidney disease, diabetes, autoimmune disease, or a multiple pregnancy. These factors do not determine who will become unwell. Postpartum symptoms and a blood-pressure measurement guide urgency, not whether someone seemed well during pregnancy.

Contact your maternity unit promptly for a new persistent headache, visual disturbance, pain beneath the ribs, sudden nausea or vomiting, rapid swelling of the face or hands, breathlessness, or feeling acutely unwell after delivery. If you have a home blood-pressure monitor, record the reading, but do not delay care to repeat measurements.

Assessment may include repeated blood-pressure checks, urine testing, blood tests, oxygen measurement, and a review of symptoms and medicines. Treatment can involve blood-pressure medicine, medicine to reduce seizure risk, fluid management, and hospital monitoring. Continue prescribed medication unless the treating clinician changes it.

Call local emergency services for a seizure, collapse, severe breathing difficulty, chest pain, new confusion, weakness on one side, trouble speaking, loss of vision, or a sudden severe headache. Do not drive yourself. Postpartum pre-eclampsia can affect the brain, lungs, liver, kidneys, and clotting, so rapid hospital assessment matters.

Ask how and when to check your blood pressure, which symptoms override the planned appointment, and whether medication is compatible with breastfeeding. Clarify who will review test results, how long monitoring should continue, and what this episode means for future pregnancy care and long-term blood-pressure checks.
