Causes of lower abdominal pain during sex

Pain felt deep in the pelvis may be triggered by penetration or certain positions. Possible causes include endometriosis, adenomyosis, ovarian cysts, fibroids, pelvic inflammatory disease, pelvic-floor muscle tension, bladder or bowel problems, and inadequate arousal or lubrication.

Pain at the vaginal opening differs from deep lower abdominal pain. Burning or surface pain can accompany dryness, irritation, infection, or tight pelvic-floor muscles. Deep pain may occur when the cervix or another tender pelvic structure is contacted.

Endometriosis and adenomyosis can present with pain during sex as part of a recurring pattern that may include painful periods. Pelvic inflammatory disease can present with deep pain plus unusual discharge, bleeding after sex, fever, or pelvic tenderness. An ovarian cyst may cause pain concentrated on one side.

Stop any activity that hurts; consent can be withdrawn at any point. When comfortable, try slower movement, shallower penetration, a different position, more time for arousal, or a compatible lubricant. These adjustments may reduce pressure but do not replace assessment when pain recurs.

Keep notes on whether pain is at the entrance or deep inside, which positions trigger it, and its relationship to periods. Include discharge, bleeding, urinary symptoms, bowel symptoms, and pregnancy possibility. A clinician may suggest pelvic examination, infection testing, pregnancy testing, ultrasound, or pelvic-floor assessment based on those details.

Get urgent help for sudden severe pelvic pain, fainting, marked dizziness, shoulder-tip pain, heavy bleeding, fever with worsening pain, or pain with a positive pregnancy test. Ectopic pregnancy can present with pelvic pain and bleeding and requires rapid assessment.

Book a medical visit when pain returns, makes penetration difficult, continues after sex, or comes with discharge, bleeding, urinary trouble, painful periods, or bowel changes. If sex was not consensual or you feel unsafe, seek emergency or specialist support in a setting where you can speak privately.

Ask whether the pain pattern suggests a pelvic condition, infection, dryness, or pelvic-floor tension; which examination or tests would clarify it; and how to manage discomfort while the cause is investigated. Tell the clinician if examination is painful or anxiety-provoking so the pace and approach can be adjusted.
