Some people have primary hypertension, where several influences gradually raise pressure without one identifiable disease. Family tendency, excess body weight, salty food, low activity, alcohol, smoking, stress, and poor sleep can contribute. Secondary hypertension has a specific trigger, such as chronic kidney disease, narrowing of an artery supplying a kidney, thyroid or adrenal problems, or obstructive sleep apnoea. Certain pain medicines, decongestants, steroids, hormonal medicines, and recreational substances can also raise readings. High blood pressure often causes no noticeable symptoms. Untreated hypertension can strain the heart and blood vessels, increasing the risk of problems such as heart failure, stroke, transient ischaemic attack, or angina. These complications are not proof of the cause of a particular reading.

Those symptoms can signal an emergency affecting the heart, brain, or blood vessels. A high reading without symptoms still needs prompt clinical advice when it remains elevated on repeat checks.
