Causes of gout in the knee

Short answer
Knee gout is caused by urate crystals in the joint provoking an abrupt inflammatory reaction.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Cause
- Urate crystals activate inflammation inside the knee joint.
- Movement
- Swelling can make bending and weight-bearing difficult.
- Diagnostic check
- Joint fluid can be examined for crystals and infection.
The cause of gout in a knee
The knee may become hot, swollen, very tender, and difficult to bend or bear weight on. Gout commonly presents with acute joint pain and swelling, but these symptoms can also occur with infection, injury, or another type of arthritis.
Why crystals form and attacks begin
The body produces urate while breaking down purines and relies mainly on the kidneys to remove it. Persistently raised urate allows crystals to accumulate. Kidney disease, family tendency, obesity, alcohol, some medicines, and certain health conditions can shift this balance.
A flare may emerge during dehydration, illness, fasting, after surgery, or following a marked change in urate. The trigger and the underlying urate accumulation are different parts of the process, so avoiding one suspected food does not necessarily prevent further knee attacks.
Managing an acutely painful knee
Reduce weight-bearing, support and elevate the leg, and cool the knee with a wrapped cold pack. Seek clinical advice promptly for appropriate anti-inflammatory treatment. The safest option depends on kidney function, digestive problems, heart health, and medicines already being taken.
Continue prescribed long-term urate-lowering medicine unless the prescriber directs a change. When the flare eases, gradually restore comfortable knee movement. A follow-up can address repeat attacks, urate control, medication contributors, and lifestyle factors without blaming a single meal.
When knee symptoms are urgent
A first episode of a hot, markedly swollen knee should be assessed the same day. Fever, chills, recent joint surgery or injection, broken skin, immune suppression, or feeling very unwell increases concern about joint infection, which cannot be safely excluded from appearance alone.
Urgent evaluation is also appropriate after significant injury, with visible deformity, or when the foot below the knee is cold, pale, numb, or weak. Persistent locking, instability, or swelling that does not settle merits a scheduled examination.
What to ask at the consultation
Ask whether fluid should be removed from the knee to look for crystals and infection, which flare medicine is compatible with your conditions, and when urate should be measured. If attacks have recurred, discuss a target-based prevention plan and how treatment will be monitored.

Questions people ask
Can gout affect only one knee?
Yes. A flare can involve a single knee, although gout may affect different joints at other times.
Does knee gout come from an injury?
An injury may sometimes precipitate inflammation where crystals are present, but gout originates from urate crystal deposition rather than a torn or bruised structure.
Why might a clinician take fluid from the knee?
Examining joint fluid can identify urate crystals and help check for infection, an important alternative cause of a hot swollen knee.
