Why gout can affect the hands

Urate can collect when the kidneys do not clear enough of it or when production exceeds clearance. Hand involvement may appear after gout has affected other joints, but any new hot, swollen hand joint deserves assessment because infection and other forms of arthritis can look similar.

Contributors and flare triggers

Reduced kidney function, inherited tendency, obesity, alcohol, and certain medicines can increase urate burden. Dehydration, an acute illness, surgery, or a sudden change in urate may provoke inflammation around existing crystals. A trigger starts a flare; it is not necessarily the underlying reason crystals formed.

Long-standing deposits can form firm nodules called tophi around finger joints, tendons, or the outer ear. In the hands, pain and swelling may interfere with gripping, writing, dressing, or opening containers even when only one joint is involved.

Protecting the hand during a flare

Remove rings promptly before swelling increases. Rest the painful joint, elevate the hand, and apply a wrapped cool pack without pressing directly on tender skin. Avoid forceful gripping and do not attempt to drain or squeeze a lump.

A clinician can select flare medicine after considering kidney function, ulcers, cardiovascular conditions, and interacting medicines. Keep taking prescribed urate-lowering treatment unless specifically advised otherwise. Recurrent hand attacks may warrant a plan that lowers urate steadily and monitors response.

Signs that need prompt medical help

Obtain same-day care for rapidly increasing redness or swelling, fever, chills, pus, a nearby cut or bite, red streaks up the arm, or severe illness. These signs raise concern for infection in a joint or surrounding tissues.

Seek urgent assessment if a swollen finger becomes blue, pale, cold, or numb, or if a ring cannot be removed. Book a non-urgent review when stiffness persists between attacks, nodules break through the skin, or hand function is progressively limited.
