Cervical myelopathy develops when the spinal cord in the neck is compressed, commonly by age-related narrowing but sometimes by other structural problems. Changes in discs, joints, ligaments, or bone can reduce the space around the cord. Less commonly, injury, a mass, infection, or an inflammatory process can create compression.
How the spinal cord becomes compressed
Cervical spondylosis can present with spinal canal narrowing as discs lose height, joints enlarge, bone spurs form, or supporting ligaments thicken. A naturally narrow canal leaves less reserve when these changes occur. A large disc protrusion or displacement after trauma may also press on the cord. Because the spinal cord carries signals to the arms, trunk, and legs, compression can affect hand dexterity, balance, walking, strength, sensation, and bladder or bowel control rather than causing neck pain alone.
What evaluation involves
Arrange medical review for progressive hand clumsiness, dropping objects, unsteady walking, limb heaviness, or altered sensation. The clinician may check reflexes, strength, coordination, gait, and fine hand movements before deciding whether neck imaging is needed. Avoid forceful neck manipulation until spinal cord compression has been assessed. Treatment depends on the cause, severity, progression, overall health, and the amount of pressure shown on imaging.
When to get urgent help
Seek urgent assessment for rapidly worsening weakness, new inability to walk safely, or sudden loss of hand function. New loss of bladder or bowel control, numbness around the genitals or buttocks, or severe neurological symptoms after a neck injury requires emergency care. Fever with severe neck pain and neurological change also warrants immediate attention. These warning signs reflect possible spinal cord compromise and should not wait for a routine appointment.
