Why does pain happen during ovulation?

A small amount of follicular fluid or blood can contact the abdominal lining and cause a temporary ache or twinge. The feeling is commonly low in the pelvis on the side of the active ovary.

A predictable episode between menstrual periods that fades on its own can fit ovulation pain. It may be sharp at first and then become an ache, and it does not have to happen every cycle.

Similar pain is not automatically caused by egg release. Ovarian cysts can present with pelvic heaviness or one-sided pain, and endometriosis can present with recurring pelvic pain related to the cycle. Infection, urinary conditions, bowel conditions and pregnancy complications are other possibilities a clinician may consider.

For a mild episode that matches a previously assessed pattern, try warmth over the lower abdomen, rest and fluids. Check with a pharmacist or clinician before taking pain relief if pregnancy is possible or if you have a medical condition, allergy or regular medicine that affects your choices.

Note the date in your cycle, the pain's location and character, any vaginal bleeding or discharge, and what made it better or worse. If the same pattern limits work, sleep, exercise or sex, make a routine appointment rather than repeatedly treating it yourself.

Get urgent medical help if pelvic pain is sudden or severe, or accompanies faintness, collapse, shoulder-tip discomfort, heavy vaginal bleeding, fever, persistent vomiting or marked abdominal swelling. These features can occur with conditions that need rapid assessment.

Take a pregnancy test if pregnancy is possible. Pain with a positive test, unusual bleeding, dizziness or weakness requires prompt evaluation to rule out an ectopic pregnancy. New pain that persists or progressively worsens also warrants a clinical review.

Tell your clinician how the pain relates to periods, sex, urination and bowel movements. Ask which diagnoses fit the pattern, whether examination or imaging would add useful information, and whether hormonal treatment is appropriate if you are not trying to conceive.
