The short answer

This mid-cycle discomfort is sometimes called mittelschmerz. It may feel like a dull ache, pressure or brief sharp pain low on one side of the abdomen, and the side can change between cycles.

How to interpret the pattern

Pain linked to ovulation generally appears around the middle of a menstrual cycle and settles without steadily worsening. Mild spotting or clear, stretchy vaginal discharge may occur at the same time.

Pain at mid-cycle can also have another cause. Endometriosis can present with pelvic pain that follows a menstrual pattern, while ovarian cysts can present with one-sided pelvic pressure or pain. Bowel, urinary and pregnancy-related problems may feel similar, so timing alone cannot confirm the cause.

Managing mild, familiar pain

Rest, a warm pack and gentle movement may help if the discomfort is mild and resembles your usual pattern. A pharmacist or clinician can advise which pain medicine is suitable for you, particularly if you could be pregnant, take other medicines, have stomach or kidney problems, or have been told to avoid anti-inflammatory medicines.

Record when the pain starts, which side is affected, how long it lasts, associated bleeding or discharge, and whether it disrupts normal activity. A cycle diary can help a clinician distinguish a recurring ovulation pattern from symptoms needing investigation.

When to get medical help

Arrange a medical review if the pain is new, keeps returning, lasts beyond the expected mid-cycle window, becomes more intense, or comes with pain during sex, urination or bowel movements. Persistent symptoms deserve assessment even when they remain manageable.

Seek urgent care for sudden severe pelvic pain, fainting, dizziness, shoulder-tip pain, heavy bleeding, fever, repeated vomiting or a rigid or swollen abdomen. If pregnancy is possible, pelvic pain with bleeding or feeling faint needs prompt assessment because an ectopic pregnancy must be excluded.

Yes. The ovary releasing an egg can differ from cycle to cycle, so discomfort may occur on a different side or sometimes not occur at all.

Does ovulation pain prove that I am fertile?

Could endometriosis cause pain around ovulation?
