What the change can mean

Blood pressure normally adjusts when posture changes. In some people, the response is stronger than expected, so a reading rises after moving from sitting or lying to standing. Contributing factors may include age-related changes, autonomic nerve dysfunction, kidney problems, excess body weight, sleep-disordered breathing, or existing Hypertension. A single reading cannot explain the cause: technique, recent activity, stress, caffeine, pain, and the timing of medicines can all affect measurements. Orthostatic hypertension is different from the blood-pressure drop that can cause light-headedness on standing.

How it is assessed

Arrange a GP assessment for repeated high readings or symptoms after standing. Rest quietly before measurement, use a properly fitting cuff, and keep the arm supported; your clinician may compare readings after lying or sitting with readings after standing. Bring a list of medicines, supplements, caffeine use, sleep symptoms, and home readings with the posture and time recorded. Do not change blood-pressure medicine without advice. Treatment focuses on the overall pattern and any underlying problem, such as Chronic Kidney Disease, Obesity, or Obstructive Sleep Apnoea.

When to seek urgent care

Call emergency services for chest pressure, severe breathlessness, fainting, new weakness or numbness on one side, trouble speaking, sudden vision loss, or a severe unusual headache. These symptoms can indicate an emergency such as Angina, Stroke, or a Transient Ischaemic Attack and should not be judged by a home blood-pressure reading alone. Seek prompt review for repeated very high readings without symptoms, especially if you have known heart or kidney disease.
