What causes hydrosalpinx?

A previous pelvic infection is an important cause, particularly when it leads to pelvic inflammatory disease. Scarring may also follow earlier pelvic or tubal surgery, endometriosis, or inflammation around the reproductive organs. The original event may have caused mild symptoms or occurred long before the hydrosalpinx is discovered.

A healthy fallopian tube helps collect an egg and provides a route toward the uterus. With hydrosalpinx, the swollen, blocked tube cannot perform that function normally. One or both tubes may be involved, so the effect on natural conception depends partly on the other tube and on other fertility factors.

Many people have no obvious symptoms, and the finding may first appear during fertility tests or pelvic imaging. Others experience ongoing or intermittent lower abdominal or pelvic discomfort, unusual vaginal discharge, or pain that changes around menstruation. Hydrosalpinx itself is not diagnosed from symptoms alone.

A fertility specialist or gynaecologist will review past infections, operations, endometriosis symptoms and pregnancy history. Ultrasound can sometimes show a swollen tube. A dye test of the uterus and tubes may clarify whether a tube is open, while laparoscopy may be considered when diagnosis or surgery is needed.

Management depends on symptoms, whether the other tube is healthy, and your plans for pregnancy. Antibiotics treat an active infection but do not remove established scar tissue. Before IVF, a specialist may discuss closing off or removing the affected tube; salpingectomy is one surgical option. The benefits and surgical risks should be considered for your individual situation.

Seek urgent medical care for severe or worsening pelvic pain, fainting, shoulder-tip pain, heavy bleeding, or pain with a positive pregnancy test or possible pregnancy. These symptoms require prompt assessment for urgent causes, including an ectopic pregnancy.

Fever, vomiting, feeling acutely unwell, or new pelvic pain with foul-smelling discharge can indicate infection and also needs timely evaluation. Hydrosalpinx found on a planned scan without acute symptoms can usually be discussed through a scheduled specialist appointment.

Ask whether one tube or both appear affected, how certain the diagnosis is, and whether further imaging is useful. If pregnancy is a goal, discuss the condition of the other tube, whether natural conception remains possible, how hydrosalpinx may influence IVF planning, and the advantages and drawbacks of surgery before treatment.
