What happens in cholera
Vibrio cholerae reaches the small intestine and produces a toxin that makes the bowel release large amounts of water and salts. This can lead to frequent, watery stools and rapid dehydration. Vomiting may add to the loss. The infection spreads when faecal material contaminates drinking water, food, hands, or surfaces. A person can become dangerously unwell even when abdominal pain is not prominent, so the amount of fluid lost matters more than how severe the cramps feel.
What to do
Start oral rehydration solution promptly if cholera is suspected and the person can drink. Give frequent small sips and continue breastfeeding when relevant. Use treated water and prepare food safely; wash hands with soap after using the toilet and before eating or cooking. Avoid preparing food for others while diarrhoea continues. Medical teams may test a stool sample and decide whether additional treatment is needed. Severe dehydration may require IV fluid therapy in a clinic or hospital.
When to seek care
Seek urgent care for very watery diarrhoea, repeated vomiting, inability to drink, faintness, confusion, extreme thirst, a dry mouth, or very little urine. Children, older adults, and people already weakened by illness can deteriorate quickly. Emergency assessment is also needed if the person becomes difficult to wake or cannot sit or stand safely. Keep replacing fluids while arranging help.
