What “cast cement” means in fracture care

Confirm the exact product and purpose with the treating team. Bone cement used during some operations is different from the rigid outer material of an ordinary fracture cast.

Padding protects the skin, while plaster of Paris or fibreglass cast material forms a firm shell around the limb. The cast limits movement so the fracture can remain in the position chosen by the clinician. A splint may be selected initially when swelling needs room.

Some displaced fractures undergo closed reduction before casting. Alignment and stability influence whether a cast is suitable; an unstable or open injury may instead need fracture fixation surgery. An ankle fracture and a distal radius fracture place different demands on a cast, so weight-bearing and movement instructions are individual.

Keep the cast clean and follow its specific water-exposure instructions. Do not push objects, powders or creams underneath it, trim its edges yourself or place weight through it unless the treating team has allowed this.

Elevate the limb as advised during swelling and move uncovered fingers or toes comfortably. Check the skin at the cast edges and compare colour, warmth, sensation and movement with the other side. Attend planned reviews because swelling can fall and fracture position may need reassessment.

Seek urgent help if pain becomes severe despite elevation and prescribed relief, the cast feels progressively tighter, or exposed digits become numb, weak, cold, pale or blue. Rapid swelling or inability to move the fingers or toes also requires immediate assessment.

Contact the cast service for a cracked, soft, wet, loose or foul-smelling cast, fluid coming through it, burning pressure, worsening rubbing or an object trapped inside. Fever or increasing redness near an edge warrants prompt medical advice.

Confirm the material used, whether the cast may get wet, and whether you may bear weight or drive. Ask what movement is encouraged, when alignment will be checked, and whom to call if the cast becomes damaged or tight.
