Carpal tunnel syndrome in pregnancy

Pregnancy-related fluid retention can reduce space inside the carpal tunnel. Symptoms may involve one or both hands and can be more noticeable at night or on waking.

The median nerve supplies feeling to the thumb, index finger, middle finger and part of the ring finger, and it supports some thumb movements. Compression can therefore feel like pins and needles, burning, aching or reduced sensation in that distribution. A hand may also feel clumsy when fastening clothes or picking up small items.

Not every painful or numb hand during pregnancy is carpal tunnel syndrome. Pain mainly on the thumb side of the wrist during lifting can follow a tendon pattern, while symptoms extending from the neck or involving the little finger may point elsewhere. An examination can distinguish these patterns.

Try to keep the wrist straight during sleep, feeding, phone use and household tasks. A correctly fitted neutral wrist splint may limit night-time bending; stop using it and seek fitting advice if it causes extra pressure, swelling or altered sensation. Remove rings if hand swelling makes them tight.

Change hand positions, share loads between both hands and avoid prolonged gripping when symptoms flare. Before using any pain medicine or anti-inflammatory product during pregnancy, check with your maternity clinician, doctor or pharmacist and name the exact product you are considering.

Tell your maternity clinician about new or troublesome hand symptoms, especially when numbness becomes constant, grip weakens, objects are repeatedly dropped or normal activities are difficult. A clinician can examine the nerve and discuss pregnancy-appropriate symptom management.

Seek urgent maternity advice if sudden swelling of the hands occurs with a severe headache, visual disturbance, pain beneath the ribs, vomiting or feeling very unwell. Those features require pregnancy assessment and should not be attributed to carpal tunnel syndrome. Urgent care is also needed if a hand becomes pale, cold or difficult to move.

If weakness is present, ask how strength will be monitored and when further testing or specialist assessment would be considered.
