What should you know about carbuncle of perineum?

A carbuncle of the perineum is a painful cluster of connected boils caused by infection around hair follicles, and it may need prompt medical assessment because this area can become irritated and the infection can spread. It can look like a swollen, red, tender lump or group of lumps, sometimes with several points that drain pus. The perineum is the skin between the genitals and anus, so keeping the area clean and avoiding pressure or friction can help while you arrange care.

A carbuncle is deeper and broader than a single boil because nearby infected follicles join beneath the skin. Bacteria can enter through a small cut, irritated follicle, shaving injury, or ongoing rubbing and moisture. Tenderness, warmth, swelling, and drainage are common features. A clinician may need to distinguish it from an abscess, an inflamed cyst, hidradenitis suppurativa, or another condition affecting the groin. Do not squeeze, pierce, or shave over the lump: pressure can push infection into surrounding tissue and can spread bacteria to other skin.

Arrange a medical assessment, especially if the lump is enlarging, very painful, draining, or making sitting or walking difficult. Until then, wash gently with water and mild cleanser, pat dry, and wear loose, breathable underwear. A clean warm compress held against the area can ease discomfort; stop if it increases pain or irritation. Cover any drainage with clean gauze, wash your hands after touching the area, and do not share towels or razors. A clinician may recommend antibiotics for a skin infection, drainage of a collection of pus, or both, depending on the examination.

Seek urgent medical care if redness or swelling is spreading quickly, pain is severe or rapidly worsening, or you develop fever, chills, marked weakness, dizziness, confusion, or vomiting. Urgent assessment is also needed for skin that becomes dark, grey, or blistered, a foul smell, numbness, or pain that seems much greater than the visible skin change. Do not wait for a carbuncle near the anus or genitals to drain on its own if you feel unwell. People with diabetes, a weakened immune system, or medicines that suppress immunity should contact a clinician promptly because infections may worsen without obvious early changes.

Ask whether the lump needs drainage, whether a swab or other test is appropriate, and which warning signs should prompt an emergency visit. Tell the clinician when it began, whether it has changed size, whether you have had similar lumps, and which medicines or health conditions may affect healing. You can also ask how to keep the area protected, when to resume shaving or exercise, and how to reduce the chance of passing bacteria to household members.
