How the treatment works

A treatment team collects some of your T cells, a type of immune cell, and modifies them so they can recognise a marker on myeloma cells. The cells are grown and returned through an infusion after preparatory chemotherapy. The process includes assessment, cell collection, manufacturing, preparation, infusion, and close monitoring.

Eligibility is individual. The team considers which treatments you have already received, whether the myeloma is responding, your organ function, infections, mobility, and whether you have support near the treatment centre. Another treatment may be used while the cells are being prepared.

Preparing for assessment and recovery

Bring a complete treatment history, current medicines, allergy details, and information about recent infections to the cellular-therapy appointment. Arrange transport and a responsible caregiver if the centre requests one. Follow the team's instructions about vaccines, infection precautions, driving, and staying close enough for review after the infusion.

Do not stop cancer medicines or start supplements without checking with the treating team. Ask who to call day or night, where urgent assessment will happen, and how your blood counts and myeloma will be monitored.

Symptoms that need prompt action

Contact the treatment team immediately after CAR T-cell therapy for fever, chills, breathing difficulty, dizziness, a racing heartbeat, new confusion, unusual sleepiness, trouble speaking, weakness, shaking, or a seizure. These can signal an inflammatory reaction, a neurological effect, or infection and require rapid assessment.

Seek urgent help as well for uncontrolled bleeding, severe chest pain, fainting, or rapidly worsening symptoms. Use the emergency plan supplied by the cellular-therapy centre and tell emergency staff that you recently received CAR T cells.

Ask which myeloma marker the treatment targets, why it is being considered now, what could delay cell collection or infusion, and which alternative treatments remain available. Clarify the monitoring schedule, caregiver responsibilities, fertility considerations, infection precautions, and the plan if the myeloma returns.

Is CAR T-cell therapy the same as a stem cell transplant?

No. CAR T-cell therapy modifies your T cells to recognise myeloma, while an autologous transplant returns your own blood-forming stem cells after intensive treatment.

Will I need chemotherapy before the CAR T-cell infusion?

Preparatory chemotherapy is commonly used to make space for the infused cells. Your team will explain the medicines and timing selected for you.

Why is close monitoring needed afterward?

Inflammatory reactions, neurological effects, low blood counts, and infections can develop after infusion. Early recognition helps the team assess and manage them promptly.
