The short answer

Sciatica describes nerve-related pain that travels from the lower back or buttock into the leg. It can arise when a nerve root is irritated or compressed by conditions that are not fully under a person's control.

What prevention can realistically achieve

Prevention focuses on keeping the back and hips capable of handling everyday loads and reducing sudden jumps in physical demand. It cannot remove every cause. A herniated disc, spinal narrowing, age-related changes, vertebral slippage or irritation around the piriformis muscle may still produce symptoms.

A previous episode does not mean every back twinge will become sciatica. Nerve-related symptoms more often include pain, tingling, numbness or weakness following a path into the leg rather than pain confined to the lower back.

Ways to lower avoidable risk

Break up long periods of sitting with comfortable movement. Build walking, strength and mobility work gradually, allowing your body to adapt before increasing load. When lifting, keep the object close, use the hips and legs, and turn with the feet instead of twisting while carrying.

At work, vary positions and arrange frequently used items so repeated reaching or awkward handling is limited. During a flare, stay gently active as tolerated rather than remaining in bed for long periods. A physiotherapist can tailor exercise when repeated episodes, weakness or a demanding job make generic advice unsuitable.

When symptoms need care

Seek emergency assessment for new loss of bladder or bowel control, numbness around the genitals or anus, or severe or worsening weakness in the legs. These symptoms can indicate major nerve compression and should not wait for a routine appointment.

Arrange a clinical review when radiating pain persists, repeatedly returns, limits walking or sleep, or comes with increasing numbness or weakness. Assessment can identify whether the pattern fits sciatica and guide activity, medication or further investigation.

Ask which movements appear sensitive, what strengthening is appropriate, how to adjust your work setup and how quickly to resume heavier tasks. If episodes recur, ask whether examination suggests a particular source and whether imaging would meaningfully change management.
