Can loop excision of the cervix cause cervical insufficiency?

The possibility is more relevant when a larger amount of cervical tissue was removed or the procedure was performed more than once. Your individual history and the appearance of the cervix during pregnancy help guide care.

The cervix is the lower part of the womb and normally remains closed during pregnancy. Cervical insufficiency means it becomes shorter or starts opening too early, sometimes without painful contractions. This can raise the chance of pregnancy loss later in pregnancy or birth before the baby is ready.

Loop excision removes abnormal cells from the cervix using a thin heated wire loop. Removing tissue may reduce the cervix’s structural support. A previous loop excision is therefore useful information for the maternity team, but it does not by itself confirm that the cervix is insufficient.

Tell your obstetrician or midwife about every loop excision, ideally at the booking appointment. If available, bring the procedure report because the amount of tissue removed can affect the monitoring plan. Also mention any previous late pregnancy loss, early birth or cervical stitch.

Your maternity team may offer a transvaginal ultrasound to measure cervical length. If the cervix is short or begins to open, options may include closer ultrasound monitoring, vaginal progesterone or a cervical stitch called cerclage. The appropriate choice depends on the scan findings, pregnancy history and whether symptoms are present; a stitch is not automatically needed after loop excision.

During pregnancy, contact your maternity unit promptly for watery fluid leaking from the vagina, vaginal bleeding, regular tightenings or cramps, new pelvic pressure, or a persistent low backache. These symptoms need assessment because they can occur when the cervix is changing, the membranes have ruptured or labour is beginning early.

Cervical shortening can also occur without noticeable symptoms, so attend any recommended scans even when you feel well. If you are uncertain where to go, call your maternity triage service or the emergency department for immediate direction.

Ask how much cervical tissue was removed, whether your previous pregnancy history changes your risk, and whether cervical-length scans are appropriate. You can also ask which symptoms should trigger a call to your local maternity unit and how progesterone, cerclage and continued monitoring would be considered if the cervix shortens.
