Can cervical spondylosis cause cervical myelopathy?

Cervical spondylosis refers to degenerative changes in the neck, whereas cervical myelopathy is impaired spinal cord function. A person can have spondylosis without myelopathy, but cord compression from those changes can produce it.

Discs can lose height and bulge, joints can enlarge, and bone spurs or thickened ligaments can reduce the space available for the spinal cord. Compression may disrupt signals travelling between the brain and the arms, trunk, and legs.

Possible features include hand clumsiness, difficulty with buttons or handwriting, numbness or weakness in the hands or arms, an unsteady or stiff-legged walk, and reduced balance. Neck pain may be mild or absent, so the pattern of limb and walking changes matters during assessment.

Book a medical assessment if you develop progressive hand coordination problems, limb weakness, altered walking, or repeated unexplained falls. The clinician may test strength, sensation, reflexes, fine hand movement, balance, and gait. Management is guided by symptoms, neurological findings, imaging, and change over time. Rehabilitation may address mobility, balance, and safe daily activity in selected cases, but exercises do not remove structural spinal cord compression. A spine specialist may discuss surgery when compression is causing meaningful or progressive cord dysfunction, with the aim of relieving pressure and limiting further neurological decline.

Seek urgent medical care for rapidly worsening arm or leg weakness, a sudden major decline in walking or balance, new loss of bladder or bowel control, or numbness around the groin or seat area. These changes can indicate significant neurological compromise and should not wait for a routine physiotherapy appointment.

You might ask which examination findings point to spinal cord involvement, where imaging shows compression, whether your function is changing, which activities or neck movements require modification, and whether review by a spine surgeon is appropriate. Also clarify which new symptoms should trigger urgent reassessment.
