Calf pain in the third trimester

Short answer
Calf pain in the third trimester may come from cramp or muscle strain, but one-sided swelling, warmth or redness needs urgent assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Urgent pattern
- New pain with one-sided calf swelling, warmth, redness or increasing tenderness.
- While awaiting assessment
- Do not massage a calf when a blood clot is a concern.
- Medication
- Check pain-relief choices with your maternity clinician or pharmacist.
How should you respond?
Pregnancy increases concern about a blood clot when calf symptoms are new or affect one leg differently. Contact your maternity team or an urgent medical service promptly for advice if that pattern appears; do not massage the area while waiting for assessment.
Possible explanations in late pregnancy
A brief, sudden tightening that releases can fit muscle cramp. Calf muscles may also become sore after changed walking mechanics, unfamiliar activity or a minor strain. Achilles tendinitis can present as lower-calf or heel pain that is aggravated by push-off.
A blood clot in a deep leg vein can present with pain, swelling, warmth, redness or tenderness, commonly on one side. Symptoms alone cannot reliably separate a clot from a muscle problem, so pregnancy-specific clinical assessment matters when these warning signs occur.
Gentle measures for uncomplicated discomfort
If there is no swelling, redness, warmth or sudden severe pain, change position and move the ankle slowly through a comfortable range. During a simple cramp, easing the knee straight and gently drawing the toes upward may relax the muscle; stop if pain becomes sharp.
Wear stable, comfortable shoes and break up long periods in one position with movement approved by your maternity clinician. Check with that team before using pain medicine or starting hands-on treatment, because suitability depends on your pregnancy and health history.
When urgent care is needed
Seek urgent assessment for new one-sided calf pain accompanied by swelling, warmth, redness, increasing tenderness or a visibly larger leg. Do the same for unexplained persistent pain, particularly after a period of reduced mobility or if you have had a clot before.
Call emergency services for sudden difficulty breathing, chest pain, coughing blood, collapse or faintness with leg symptoms. These signs can accompany a clot that has travelled to the lungs and should not be monitored at home.
Questions for your maternity clinician
Describe whether one or both calves hurt, any difference in size or temperature, when the pain began, and what you were doing beforehand. Ask whether you need same-day examination, which movements are appropriate, and which pain-relief options fit your pregnancy and other medicines.

Questions people ask
Can calf cramp happen in the third trimester?
Yes, a brief involuntary muscle tightening can occur. A cramp generally releases, whereas persistent one-sided pain with swelling, warmth or redness requires urgent assessment.
Can I massage a sore calf during pregnancy?
Do not massage a newly painful calf that is swollen, warm, red or unusually tender. Seek urgent medical advice first so a blood clot can be assessed.
What should I tell my maternity team?
Explain when the pain began, whether it affects one leg, and whether there is swelling, warmth, redness, breathlessness or chest pain. Mention reduced mobility and any previous blood clot.
